Please read over the following forms before the initial session.  You may wish to print out a copy for your own records. 

The New Patient Information form and the last page of the informed consent agreement form will need to be completed and signed. The Texas Notice of Privacy form is for your own reference and does not need to be signed. 

If you are 18 years old or older, please complete the adult new patient form.  Likewise, if you are the parent of an adolescent who is 17 years old or younger, please complete the adolescent new patient form.  You may choose to print these forms out and complete them at home or, alternatively, I will provide you with copies during your first visit.
